Three Ways to Sign
Bringing your signed forms to either office is the way we prefer: our front desk can check them with you there and then. If that isn't easy, the client portal needs no printer at all.

Print them, sign them at home, and bring them in — to your appointment, or any time we are open. Our front desk will go through them with you and answer anything you are unsure about. This is the way we prefer.

Sign online in the client portal
Nothing to print and nothing to scan. You read each form on screen, sign it there, and it reaches your record straight away — no journey and no post.

If it is easier to mail them, print and sign them and send them to our mailing address. Allow a few days for the post.
Where to send them
Please don't email a signed form back to us. Email isn't secure, and a signed consent carries your name and health information. Use the portal, bring it in, or post it instead.

Your Rights and Our Notices

Other Forms
These are signed one at a time, for a particular reason, so they aren't part of a packet.
Release of Information
Sign one of these when you want us to share information with someone outside Confianza Health — a doctor, a school, a court, a family member. You sign a separate one for each person or organization, and you can cancel it at any time.
Social Determinants of Health
Questions about housing, food, transportation, safety and the other things outside the therapy room that affect how you are doing.
Safety Plan
A plan for getting through a hard moment, usually filled in with your clinician: your own warning signs, the people you can reach, and the numbers to call.

Where to Send a Signed Form
This is a mailing address only, not an office you can visit. To hand a form to someone, bring it to the Avondale or Phoenix office.
You May Not Need to Print Anything
Every form on this page can be signed in the client portal instead: you read it on screen, sign it there, and it goes straight into your record. No printer, no stamp, and nothing to bring to your appointment.
