The ACE Questionnaire
Ten questions about childhood, answered only for yourself. Nothing is saved, and nothing is sent to us.
Before You Begin
This is the ten-question ACE questionnaire, for adults thinking about their own childhood. It asks directly about abuse, neglect, and what went on at home before you turned 18. Some of it is hard to read.
You can stop at any time, skip any question, and nobody will know either way. Your answers stay in this page: nothing is sent to us, nothing is saved, and closing the page clears it. There is no sign-in and nothing to submit.
If you are worried about a child who is being harmed right now, this questionnaire is the wrong tool. Call the Childhelp National Child Abuse Hotline, 1-800-422-4453, any time.
If You Need Help Now
If you are in crisis, call or text 988 any time, day or night. For Spanish, call 988 and press 2, or text AYUDA to 988. In an emergency, call 911.
The Questionnaire
Our relationships and experiences—even those in childhood—can affect our health and well-being. Difficult childhood experiences are very common.
Your answers stay in this page. Nothing here is sent to us or saved, and closing the page clears it.

If You'd Like to Talk About It
You don't need to bring anything or show anyone a number. If you want to talk about what came up, tell us you'd like to start, and we'll take it from there.
Our offices in Avondale and Phoenix are open Monday to Friday, 9am to 5pm, in English and Spanish, in person or by telehealth anywhere in Arizona.
More About ACEs
Adverse childhood experiences are one part of a person's story, not the whole of it. We offer ACE education for families, schools and community partners on how these experiences shape us and how resilience is built.
